A Couple's Sleeve Gastrectomy: Video by Dr. Hu Guangjun

Dr. Hu Guangjun walks through two laparoscopic sleeve gastrectomies on one operating day: long trocar, calibration tube, stapling and reinforcement.

A couple, one operating day, two laparoscopic sleeve gastrectomies. In this video Dr. Hu Guangjun, Chief Physician in Gastrointestinal Surgery at Cangzhou People's Hospital (Oncology Campus), takes viewers into the operating room and explains the key steps as he performs the operation. We share the English version here as a teaching aid for surgical and anaesthesia teams and for anyone who wants to see how the procedure is carried out in practice.

Key steps in the video

Extra-long trocar. For a patient with a thick abdominal wall, the team uses an extra-long abdominal trocar. Dr. Hu holds it next to a standard trocar to show the difference in length and explains why it is needed to reach the abdominal cavity.

Mobilisation. The stomach is freed along the greater curvature, including the fundus, using an ultrasonic scalpel.

Calibration tube before stapling. The assistant is asked to insert the gastric calibration tube (bougie) before the stomach is divided. The tube acts as a guide so that the remaining sleeve has a consistent width.

Stapling. A linear cutting stapler is used to shape the stomach into a banana-shaped sleeve. The removed part of the stomach is later shown as the surgical specimen.

Staple-line reinforcement. Dr. Hu then uses a barbed suture to place a seromuscular inverting stitch over the staple line, reinforcing the cut edge of the remaining stomach.

The video also includes the anaesthesia team describing how sedation, analgesia and muscle relaxation are coordinated for intubation, and a short conversation with the second patient before his operation.
